A. Information You Provide to Us

  • Creator Account Information: When you register for an account, we collect your name, email address, and password. You may also choose to create a public vanity URL for your profile.
  • Team Information: If you create or are invited to a team, we collect information about your team membership and your role within that team.
  • Form Content: We store the forms you create, including the questions, text, images, and any other content you add.
  • Media Uploads: If you upload images or other media files to use in your forms, we store these files.
  • Respondent Data: We collect and store the information that Respondents submit to your forms. This data belongs to the Creator, and we process it on their behalf.
  • Communications: If you contact us directly via email (e.g., at support@formrobin.com), we may receive additional information about you such as your name, the contents of the message, and any other information you may choose to provide.

B. Information We Collect Automatically

  • Log and Usage Data: Like most websites, we collect information that your browser sends whenever you visit our Site. This may include your IP address, browser type and version, the pages you visit, the time and date of your visit, and other diagnostic data.

  • OpenAI, L.L.C.: To provide our AI-powered form generation feature. If you use this feature, the text prompts you provide may be sent to OpenAI to generate form content.
  • SendFox: To manage and send transactional emails, such as account notifications, team invitations, and form submission alerts.

We may also share your information in the following circumstances:
